POLICE AND FIRE PENSION BOARD OF TRUSTEES REGULAR MEETING

2. DePrince, Race & Zollo in the amount of $18,870 for quarter end 3/31/21
3. Garcia Hamilton & Assoc. in the amount of $2,331.46 for quarter end 3/31/21
4. Integrity in the amount of $5,517.82 for quarter end 3/31/21
5. Sugarman & Susskind in the amount of $5,664.18 dated 4/15/21

8. Administrative fee for Plan Administrator $2758.34 monthly (auto debit)
9. Fees deducted from distribution for UBS in the amount of $3429.50 for quarter 3/31/21

11. Ciox Health invoice for copy of medical records from Advent Health for disability claim
of Javier Rodriguez dated 4/20/21 in the amount of $535.19
12. SIMED Health invoice for IME/record review of Javier Rodriguez in the amount of $1587.50
13. FPPTA Pension Fundamentals course 3/10/21 for Jermaine McKoy in the amount of $150 and
approval for CPPT Registration and Beginning course 4/20 & 4/21/21 in the amount of $1400.
B. Acknowledge/approval of retirements/contribution refunds/buy-in/stop payments for the following:
Retirement: William Amos, Social Security Option to age 64 with 100% Joint Annuitant
and 20% PLOP 3/1/21; Terminated vested member Wayne Paisley Life Only Social Security
Option to age 66 and 2 months; John Lau revision to include career incentive
Contribution refunds (interest): Potters, Hervochon, Connor, Quinn
C. Deposit in the amount of $474.57 for securities litigation settlement from Wilmington Trust
D. Submitted the declared rate of return to the City and State
