The City of Titusville established the Titusville Community Service Award in memory of and to honor Reverend Harry R. Lee for his outstanding, long-term contributions to our community. This award will be presented to a recipient at the City's annual Flag Day Ceremony on June 14, 2006.
Reverend Lee served the community as the City’s Chaplain and Employee Assistance Advisor, the Chaplain of Sertoma Club and of the Titusville Area Chamber of Commerce. By his spirit, words and actions, he personified the concept of unity and the spirit of service in the community. He set an example of community service.
Your organization is invited to submit a nominee(s) who has contributed "untold" service to our community and as a result promoted unity and harmony. Nominations may be submitted by organizations and/or individuals. Nominations must be received at the City Clerk's Office, 555 South Washington Avenue, P.O. Box 2806, Titusville, Florida 32781-2806, by Monday, April 24, 2006. Nominations can be faxed to 383-5704, or sent by e-mail to firstname.lastname@example.org (see attached form). Your help is needed! We know there are a lot of outstanding citizens who have contributed to our community and we would like to recognize their efforts. The committee will select the person who, in its judgment, has made the greatest contribution toward community service.
