APPLICATIONS WILL BE ACCEPTED BETWEEN MARCH 1, 2017 AND JUNE 30, 2017.  ANY APPLICATIONS RECEIVED OUTSIDE OF THIS PERIOD WILL NOT BE ACCEPTED.

SUBMITTAL AND ACCEPTANCE OF AN APPLICATION DOES NOT GUARANTEE ASSISTANCE, SOME APPLICANTS WILL NOT QUALIFY.  PLEASE TAKE TIME TO READY THE BELOW INFORMATION BEFORE PROCEEDING.


If you wish to apply for assistance with repairs to your home, please fill out the below information to submit your "Preliminary Intake Application."

Your participation in the program is strictly voluntary.  Please note that there are certain qualification criteria that must be met.  The program is funded with federal and state grant funds, and a lien will be placed on your property to ensure compliance with grant conditions.

Please be advised that once an application is submitted for assistance, your name and address becomes public record, and a record search will be initiated on the property and owner of record in accordance with the Program policies and procedures.

The City does not share sensitive information such as social security numbers, or other protected forms of identification and personal information associated with applications.  However, case files are made available for review to HUD and the State of Florida as part of the audit and monitoring of the City's programs.

Thank you for your interest in the Housing Rehabilitation Program.  Based upon your responses to the above questions, a determination will be made as to your eligibility for the program.  If you are determined to be eligible, your name will be placed on the "waiting list" and you will receive a follow up application in the mail once your name reaches to the top of the list; this process can take up to two years.  If for any reason you are deemed ineligible, you will be notify of such in writing.

When your name comes up on the "waiting list" you will be contacted for an office appointment. 

In the meantime, you should ensure that you and your property meets all the criteria for assistance, and there are no outstanding issues that would delay your assistance.

This program has very limited funds and is available citywide and are not developed to address any particular population or defined by any boundaries. The program is designed to address household needs and shall not be deemed to be entitlement based. Because funds are very limited, all assistance provided under this program will be based upon the owner's financial ability to make the necessary repairs without assistance.
